Abstract

Abstract

Background. Evaluating cerebral hemodynamics in servicemen with combat acubarotrauma is crucial for predicting the disease course and developing targeted treatment strategies for extra-aural disorders.

Objective. To investigate rheoencephalography (REG) parameters in servicemen with combat acubarotrauma depending on the degree of auditory analyzer impairment and the efficacy of the treatment provided.

Materials and Methods. Sixty-four servicemen with acubarotrauma were examined and divided into three groups: Group 1 – 23 patients with mild hearing loss and positive therapeutic effect; Group 2 – 22 individuals with moderate auditory dysfunction; Group 3 – 19 patients with a progressive disease course and low treatment efficacy. Cerebral blood flow was assessed using bilateral REG in the carotid and vertebrobasilar basins based on α, DCI, DSI, and RI parameters.

Results. Prominent hemodynamic disturbances dominanting in the vertebrobasilar basin were revealed in patients with acubarotrauma. A statistically significant deterioration of parameters (p < 0.05) was observed as the degree of hearing loss increased. Group 3 patients demonstrated marked vascular tone instability, profound venous outflow impairment (up to 95.61%), and a critical deficit in pulsatile blood filling (up to 100.0%). The rheographic index (RI) values were sharply reduced across all groups, regardless of the vascular tone direction, reaching a minimum in individuals with resistant and progressive sensorineural hearing loss (SNHL).

Conclusions. Combat acubarotrauma is accompanied by systemic changes in cerebral blood flow, the severity of which directly correlates with the severity of auditory dysfunction. The RI serves as a reliable objective marker for predicting the SNHL course and evaluating patients during military medical examination.
